分析1.5T核磁共振对早期鉴别诊断颞颌关节紊乱综合症的效果(1)

     【摘 要】 目的 : 探讨1.5T核磁共振对早期鉴別诊断颞颌关节紊乱综合症的效果。方法 : 选取40例颞颌关节紊乱综合症患者(64个病变颞颌关节),并同期选取30名健康体检者(60个颞颌关节),均以X线和1.5T核磁共振(MRI)检查,将病理检查结果或内镜检查结果作为对照,分析MRI对诊断颞颌关节紊乱综合症的价值。结果 : X线诊断敏感性为91.38%,特异性为86.59%,诊断符合率为88.57%;MRI诊断敏感性为100.00%,特异性为95.00%,诊断符合率为97.14%;MRI诊断敏感性、特异性和符合率均高于X线(P<0.05)。结论 : 在颞颌关节紊乱综合症中MRI诊断效果良好。

    [Abstract] Objective:To investigate the effect of 1.5T MRI on the early differential diagnosis of temporomandibular joint disorder syndrome. Methods: 40 patients with temporomandibular joint disorder syndrome (64 diseased temporomandibular joints) and 30 healthy subjects (60 temporomandibular joints) at the same time were selected, all of them were examined by X-ray and 1.5T magnetic resonance (MRI). The value of MRI in the diagnosis of temporomandibular joint disorder syndrome was analyzed by comparing the results of pathological examination or endoscopy. Results: The sensitivity of X-ray diagnosis was 91.38%,specificity was 86.59%, diagnosis coincidence rate was 88.57%. The sensitivity of MRI diagnosis was 100.00%, specificity was 95.00%, diagnosis coincidence rate was 97.14%. The sensitivity diagnosis, specificity and coincidence rate of MRI were higher than X-ray (P<0.05). Conclusion: MRI is effective in diagnosing temporomandibular joint disorder syndrome.

    颞颌关节紊乱综合症是一种常见的以侵害颞颌关节和咀嚼系统为主的口腔颌面部疾病,其致病机制不明,临床以关节局部疼痛、肩颈部疼痛、头部疼痛、下颌运动障碍以及关节弹响等为特征。目前,X线是临床检查颞颌关节紊乱综合症的有效方式之一,但长期调查发现[1],X线虽能够显示关节结节及间隙等部位,但对关节盘显示较为模糊,很容易影响临床诊断结果。而近期有学者发现[2],MRI检查能够显著提高临床鉴别诊断颞颌关节紊乱综合症的准确性。本研究针对1.5T核磁共振在颞颌关节紊乱综合症中诊断价值进行分析。

    1 资料与方法

    1.1 一般资料

    将2017年1月至2018年6月就诊于本院且经临床确诊的40例颞颌关节紊乱综合症(TMD)患者作为研究对象,TMD诊断的标准为:颞下颌关节酸胀或疼痛、关节弹响及关节运动障碍。颞颌关节病变64个,无病变16个。其中,男26例,女14例,年龄26~57岁,平均年龄(41.34±2.19)岁;同期选取30名健康体检者,60个颞颌关节,男20名,女10名,年龄25~60岁,平均年龄(41.53±2.23)岁。所有研究对象对本次研究知情且本研究获得本院伦理委员会批准。

    1.2 方法

    X线检查方法:取飞利浦X线机(电流设置100mA、电压为65kV,曝光时间0.3s),扫描位置为颚下颌关节张闭口许勒位。

    1.5T MRI扫描方法:所有受检者均采用美国GE Signa HDA 1.5T超导核磁共振扫描仪,眼眶表面线圈,取仰卧位,扫描体位同颅脑MRI检查,颞颌关节置于眼眶表面线圈中心。嘱受检者静卧勿动,尽可能避免吞咽动作,在闭口和开口两种状态下,选斜矢状扫描方位和斜冠状扫描方位,采用FSE序列T1W抑脂(TR400ms,TE30ms)、FRFSET2W(TR5580ms,TE98ms,ET24)、PDW(TR2760ms,TE10ms,ET10),所有序列FOV:120mm×120mm,矩阵256×256,层厚2mm,间距0.1mm,NSA2。, 百拇医药(龚晓萍 刘成环 李俊华 祁海峰)
