【摘要】 目的 分析膈下游离气体误诊为上消化道穿孔的原因， 进一步提高诊断的准确性。方法 回顾分析1例腹部立位平片提示膈下游离气体， 并经腹腔镜探查为误诊患者的临床资料。结果 患者经术中胰腺坏死组织清除和腹腔引流， 术后抗感染、补液治疗后痊愈出院。结论 膈下游离气体不是诊断上消化道穿孔的唯一标准， 要充分认识膈下游离气体的病因和鉴别诊断， 避免误诊。
