新生儿缺氧缺血性脑病脑实质CT值与血清NSE关系的研究(1)

     【摘要】 目的 探讨新生儿缺氧缺血性脑病(HIE)中脑CT值与神经元特异性烯醇化酶(NSE)的相关性,为诊疗提供依据。方法 收集45例HIE患儿,采用TOSHIBA Aquilion 4螺旋CT扫描脑部,并检测血清中NSE水平,同时选择同期住院均为咽下综合征患者45例作对照组。结果 HIE组与对照组比较,额叶、颞叶的CT值有明显差异(P<0.01),而枕叶与顶叶存在一定差异(P<0.05),显示HIE患儿脑损伤以额叶、颞叶为主,枕叶、顶叶次之;HIE患儿脑部CT值越低,其血清NSE值越高,两者呈直线相关。结论 HIE患儿血清NSE的水平和脑实质CT值对评估脑实质的损伤的程度有参考意义。

    The relationship between CT scan value and neuron specific enolase in neonatal hypoxicischemic encephalopathy

    【Abstract】 Objective

    To explore the relationship between CT scan value and neuron specific enolase in neonatal hypoxicischemic encephalopathy provided the basis for the diagnosis and treatment. Methods Collected 45 cases of children with HIE, used Siemens Sensation16 spiral CT scaned the brain, and detected serum NSE level, selected 45 patients with swallowing syndrome as the control group. Results HIE group compared with the control group, frontal and temporal lobes were significant differences in CT values(P<0.01), and there were some differences between occipital and parietal (P<0.05).Show brain damage in children with HIE is main in frontal and temporal lobe, the secondary was occipital lobe and parietal lobe, the lower of the brain CT value, the higher of the serum NSE value, both showed linear correlation. Conclusion There are reference significance in serum NSE level and CT scan value to assessed the extent of brain damage in neonatal Hypoxicischemic encephalopathy.

    新生儿缺氧缺血性脑病(Neonatal Hypoxicischemic Encephalopathy,HIE)是围生期窒息引起新生儿的损伤,主要为脑损伤,是引起儿童残疾和死亡的重要原因之一。因此新生儿缺氧缺血性脑病的早期、正确诊断非常重要,而影像学又是诊断的重要方式之一,其中CT更是首选的诊断方式。神经元特异性烯醇化酶(neuron specific enolase, NSE)是一种神经系统特异性蛋白质, 主要存在于神经元细胞浆中, 脑组织损伤后, NSE 从损伤的神经元漏出, 通过血脑屏障进入脑脊液和外周血液, 检测血液中NSE水平可反映神经元损伤的程度。本研究通过对HIE患儿NSE水平及头颅CT值检测,探讨血清NSE水平及CT值在HIE时的变化以及相互关系,为临床早期诊断,评价脑损伤程度及预后,并探讨HIE发病机制提供一定的理论依据 ......
